Parental Consent / Sign-up form.

This form only has to be filled if you are under 18 years of age. 

 

If you are under 18, one of your parents or a guardian must fill out the bottom of form and sign even if it is only a trial lesson. You can tick the "Test Time / membership association" if it is only a trial lesson, it is not necessary to fill in the bank details. If you pay cash for 6 months, bank details still has to be filled, in case of any fines for non attendance. Remember to bring your social security card when you come.
